Nursing Name Badge

Name badges are blue with white lettering. The logo is blue and gold. Information will be printed in white. This is the standard set-up for name badges.

You must use your LEGAL NAME as it will appear or currently appears on all of your nursing licenses, certifications, and official documents. Nicknames are not acceptable.
